AFFIRM Annual Leadership Awards Call for Nominations

The AFFIRM leadership awards recognize outstanding accomplishment by an individual or team in providing a firm direction, inspired vision and outstanding contributions within or beyond their agency or organization. Their high impact achievements help government to be more effective and efficient in delivering its mission.

This form can be used to submit an individual or group nomination. If you are submitting a group award please indicate the name of the group and the names of the group members.

The deadline to submit a nomination is midnight on Friday, April 7, 2017.

Nominations are being accepted in the following categories:

Executive Leadership in Information Technology (Civilian)
Presented for outstanding executive leadership in government-wide information technology. The AFFIRM Executive Leadership Award has been presented annually since 1979 and is the highest and most prestigious AFFIRM award.

Executive Leadership in Information Technology (DoD and Intelligence)
Presented for outstanding executive leadership in government-wide information technology. The AFFIRM Executive Leadership Award has been presented annually since 1979 and is the highest and most prestigious AFFIRM for members of the Intelligence and Department of Defense Community.

Executive Leadership Award for Industry - Large
Presented for superior leadership in business and technology within the government technology community. This award is present to an executive who has demonstrated outstanding leadership and contributions to the community within and beyond information technology.

Executive Leadership Award for Industry - Small
Presented for superior leadership in business and technology within the government technology community. This award is presented to an executive of an SBA-defined small business who has demonstrated outstanding leadership and contributions to the community within and beyond information technology.

Congressional Leadership Award
This award is presented to a member or staff member of either the House or Senate who has exemplified outstanding leadership in developing legislation, providing oversight and review or other actions which support the improvement of the effective and efficient management of information technology in the federal government.

Leadership Award in Acquisition and Procurement
Presented for leadership excellence in government information services for acquisition and procurement improvement and effectiveness for the government or for their Agency.

Leadership Award for Service to the Citizen and the Country
Presented for leadership excellence in management information technology for improved services to the citizen and on a national level for their country.

Leadership In Health Information Technology
Presented for leadership excellence in providing information technology health services government-wide or for an Agency.

Leadership in Technology Innovation
Presented for leadership excellence in innovative technology or innovation government-wide or for an Agency.

Leadership In Cyber Security
Presented for leadership excellence in protecting government mission critical data and systems government-wide or for an Agency.

Leadership in Cloud Computing
Presented for leadership excellence in implementation or management of Cloud Computing government-wide or for an Agency.

Leadership in Mobile Computing Technology
Presented for leadership excellence in implementation or management of Mobile Computing Technology government-wide or for an Agency.

Leadership in Big Data and Data Analysis
Presented for leadership excellence in implementation or management of Big Data or Data Analytics Government-wide or for an Agency.

Leadership in Digital Services
Presented for demonstrated leadership in innovative problem solving using the best of product design and engineering practices to transform government-to-government services and government-to-consumer services making them easy to use.

Special Recognition Awards
Presented for leadership excellence in various information technology areas not covered above. The leadership can be demonstrated for government-wide activities or for those in at a specific Agency. The specific title of the Special Award should be listed. Examples include:

  • Agile
  • Cloud Computing
  • Big Data
  • Mobile
  • Cybersecurity
  • Other
  • Nomination Justification

    In five (5) sentences or less, describe why the nominee is being recognized for the selected leadership award category for the calendar year 2016.

    In addition, describe how the nominee demonstrated exemplary leadership in identifying and overcoming barrier(s) and what the nominee did that led to the successful outcome. Describe the stakeholders (internal and/or external) and customers. Describe measurable benefits achieved as a result of the nominee’s leadership.

    • Nomination justifications MUST reflect achievement for the specific award category selected (see Award Categories 2017).

    • Please provide details sufficient for the awards committee to be able to adequately judge the merits of the nomination. Specificity is more important than length.

    • Submissions are limited to 2 pages. Any submissions that exceed 2 pages will not be considered.
